Continuity Experience

One of the defining experiences of Baystate Med-Peds training is our combined Internal Medicine-Pediatrics continuity clinic at Baystate Mason Square Neighborhood Health Center. Mason Square has long been an important home for Med-Peds ambulatory education and remains one of the distinctive strengths of our program.

At Mason Square, residents care for newborns, children, adolescents, adults, and families within a community health center setting. This all-ages continuity experience allows residents to develop the full scope of Med-Peds primary care while building meaningful, long-term relationships with patients over the course of residency.

Your Own Panel of Patients

As a resident, you will build and follow your own panel of patients throughout training. You will provide preventive care, acute care, chronic disease management, and care coordination across age, health status, and life stage.

Over time, residents come to know their patients not only by their diagnoses, but also by their stories, families, communities, goals, and barriers to care. This continuity is one of the places where Med-Peds training becomes especially meaningful.

Team-Based Care in a Community Health Center

Mason Square is a Patient-Centered Medical Home, where residents learn to provide comprehensive, coordinated care within a team-based model. Residents work alongside nurses, advanced practitioners, behavioral health clinicians, community health workers, pharmacists, social workers, care coordinators, and other members of the care team.

The clinic also offers access to integrated and on-site services that may include behavioral health, optometry, HIV care, TB care, nephrology, pulmonary medicine, diabetes care, developmental disabilities care, reproductive health services, and substance use treatment.

Training in this setting helps residents understand both the power and the complexity of primary care. Residents learn how to partner with patients, advocate within systems, coordinate care across specialties, and provide care that respects each patient’s needs, values, culture, and preferences.

The Heart of Med-Peds Primary Care

For many residents, Mason Square is where Med-Peds “clicks.” It is where caring across the lifespan becomes real, longitudinal, and deeply connected to community.

Residents value the opportunity to care for multiple generations within families, to develop relationships over time, and to practice full-spectrum primary care in a setting that reflects the needs and strengths of the community we serve.
