Get Your Patients The Specialty Care They Need

We value our partnerships with our referring health care providers. If you have a patient who needs to see one of the specialists at Baystate Health, we’re here to help. We offer many benefits to your patients, such as:

Refer Your Patient to a Baystate Health Provider or Practice

You can fill out a referral form and fax it to us to start the referral process for your patient. We have a form for pediatric patients and a form for adult patients.

Once you have completed the form, fax it to the fax number found on the referral form. Depending on your preference, we can either make the appointment and fax it back to your office, or we can contact your patient directly to set up their appointment.
