The weight loss (bariatric) surgery process starts with an in-depth screening. We want to make sure you're a good candidate for weight loss surgery and that you know what to expect.

The screening process starts with an information session, which can be completed by watching this informational video online. Once you have watched the entire video, please answer the screening questions on the next page. Someone from our program will reach out to talk with you about next steps. (En Español)

To move on to the next phase in the screening process, please fill out and submit our online form. Some of the questions are based on information you've learned in the video. Our team will call you in 2-4 business days to follow up.
