Integrated Home-Based Services
Baystate Wing Hospital provides home-based medical and psychiatric services for patients leaving inpatient care. This service has allowed patients to avoid repeat ED visits and inpatient stays.
We offer patients discharged with Chronic Obstructive Pulmonary Disease (COPD) and/or a substance use disorder a team of a RN and clinical social worker. The team goes to their home post-discharge and helps the patient with whatever they identify as their needs.
This includes:
- Social determinants of health
- Medication reconciliation
- Health literacy
- Health system navigation
- Advocacy
- Bridging behavioral health treatment for those awaiting a mental health appointment
Each patient receives both a nursing and behavioral health assessment in the home, addressing both aspects of health.
