Preadmission evaluation is available to all patients over age 14 who are scheduled for day stay or inpatient surgery at Baystate Medical Center. Your preadmission evaluation usually occurs 7-10 days before surgery and will help prepare you and your family for your experience before, during and after your surgical procedure.
We encourage all patients to complete a Health Care Proxy, a legal document that names the person you choose to make health care decisions for you in the event that you are unable to make your own decisions.

What to Expect
Your preadmission evaluation is determined by the surgical procedure you are having and your health history. It will include telephone screening or an on-site visit, conducted by a registered nurse. Laboratory tests, ekg or chest x-ray, may be ordered based on your medical history or surgery procedure. Your information will be reviewed by a nurse practitioner from the Department of Anesthesia.
