Preadmission Evaluation

Preadmission evaluation is available to all patients who are scheduled for day stay or inpatient surgery at Baystate Medical Center. Your preadmission evaluation usually occurs the week before surgery and will help prepare you and your family for your experience before, during and after your surgical procedure.

We encourage all patients to complete a Health Care Proxy, a legal document that names the person you choose to make health care decisions for you in the event that you are unable to make your own decisions.
