Your Own Panel of Patients
You will have your own patient panel that you will follow for four years.
Mason Square is a Patient Centered Medical Home which means that you will provide comprehensive health care for your patients, coordinating their care across our health system.
In our patient-centered care model you will focus on the whole person—understanding and respecting their needs, culture, values, and preferences—and supporting them in becoming active participants in their health care.
You will be part of a team of healthcare professionals that includes nurses, advanced practitioners, nutritionists, social workers, pharmacists, and care coordinators. Your team helps care for your patients when you are not available.
