Continuity of Care: The Foundation of Family Medicine

Greenfield Family Medicine Doctor and Pediatric PatientThe goal of Greenfield Family Medicine Residency's curriculum is to ensure that graduates are comfortable and competent in caring for patients of all ages and conditions, and in dealing with the entire spectrum of acute and chronic clinical issues that might be encountered by the members of a family unit.

The longitudinal continuity curriculum prepares you for interprofessional team-based practice in a culture of continuous quality improvement, safety, and accountability by using the behaviors of the reflective practitioner to identify opportunities for improvement in your practice environments and develop lifelong learning skills.

Greenfield Family Medicine Clinic and Building

We provide team-based comprehensive care for babies, children, and adults of all ages from diverse backgrounds and experiences. We value treating the whole person—better yet, the whole family!

From the first month of residency, you will take care of your own panel of patients. Across three years and under the guidance of faculty preceptors, you will coordinate the care of patients and families across all the ages and stages of life. We have an innovative integrated behavioral health model with a full-time mental health providers who will work in conjunction with you to optimize patient care.

In the clinic, you will become competent in a variety of in-office procedures, treat substance use with medications for opioid use disorder (MOUD), and counsel patients via traditional office visits, telehealth, and group sessions.

Your clinic will be part of a new teaching health center right across the street from Baystate Franklin Medical Center, our community hospital. Additional rotations take place across the region including some at Baystate Medical Center, our affiliated tertiary care center in Springfield, MA.
