Baystate Home Health provides high quality care, expressly tailored to meet each patients’ needs. Our home health team works together to ensure a safe and swift recovery from illness, accident, or surgery in the comfort of home.
Baystate Hospice offers medical expertise through our extensive network of caregivers to support patients facing a serious or life limiting illness. Each patient and family is cared for by our certified and experienced nurses, therapists, social workers, hospice aides, spiritual and bereavement counselors, and volunteers. This care team works together, with both the patient and family, to bring understanding, comfort, dignity and a sense of peace, as each patient journeys towards the final stage of life.
New Name Reflects Higher State of Caring
Baystate Health has consolidated our visiting nurse and hospice services to make delivery of home health and hospice care seamless and coordinated. We now have three locations that serve our community as Baystate Home Health. This new name reflects the wide range of services, support and resources we provide across western Massachusetts.
Our hard work and dedication has helped us to grow steadily into the premier home health, hospice, and palliative care agency in the region. Our patients and families can take comfort in knowing that no matter where they live, they will receive the highest quality care.
Request Services
Requests for home care or hospice services may come directly from a community resident, a physician, family, friends, clergy, hospital, rehabilitation facility, or skilled nursing facility discharge planners. Upon referral, our team works quickly and closely with each patient and attending physician to determine the best plan of care.
In order to best meet the needs of our communities, we contract with as many insurance carriers as possible. If you don’t see your plan listed, please call our intake office for more information.

Referring to Baystate
Most insurance plans Referrals for our services from physicians’ offices can also be made by downloading our referral form and faxing everything directly to our office at 888-207-3106. Please be sure to include the patients’ demographics sheets, medication list, insurance information, and face-to-face office visit note supporting the need for this referral. You will receive confirmation from our intake team once we receive your referral.
Baystate Medical Practices can refer electronically to Baystate Home Health and Baystate Hospice directly from CIS.
