Shared Decision Making and Renal Supportive Care (Phase 2) - improving communication between patients and staff

The purpose of this research study is to see if we can improve communication between patients who are receiving dialysis and the staff caring for them. We are especially interested in helping people with more severe illnesses inform their dialysis providers about goals and treatment preferences.

Subjects must have kidney failure and be undergoing hemodialysis
