The Health Questionnaire is a series of questions about your health needs, strengths and challenges. Your responses are combined to build your personalized wellness profile. It includes simple questions about lifestyle behaviors (such as diet, exercise, smoking, stress) and physical measurements (such as height, weight, cholesterol and blood pressure.) Once you complete your health screening your physical measurements are sent securely from Interactive Health, Inc. to WebMD and loaded into your Health Questionnaire for you. Taking the Health Questionnaire each year allows you to see your progress over time. It is also the “gateway” to the MyHealth Rewards program and therefore needs to be completed annually.

Yes. Your information is kept secure and confidential. The Health Questionnaire is provided by WebMD and the health screening biometrics are taken by Interactive Health Inc. Baystate Health does not see your individual responses or any personal health information. These companies only provide Baystate Health with a summary report to determine the overall health profile of the Baystate population. This information is used by the Baystate Healthy team to help plan future programs and benefits that can help employees reach their health and wellness goals
If you choose to participate in MyHealth Rewards, you are required to complete the minimum requirements: the annual WebMD Health Questionnaire and an annual Health Screening. That health screening must include a measure of all 4 biometrics: glucose, BMI/Waist, HDL or LDL and glucose. The Wellness Activities are optional.
No. You may have your health screening done through your health care provider using the Physician Screening Results form. You are required to have all four screenings: blood pressure, body mass index, HDL or LDL cholesterol and glucose.
You earn rewards just by completing a health screening and the health questionnaire. If you did not meet the individually set Target Values, you can retest one additional time during the program year, through your health care provider, using the Physician Screening Results form.
If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
