Q: Who is eligible to use the WebMD® Portal?
A: The WebMD® Portal is available to all benefits-eligible employees. In addition, the spouse, domestic partner and dependent (18 years+), who is enrolled in the BH-HNE medical plan, has access to the full-suite of tools and resources within the WebMD® Portal.
Q: Will I lose my WebMD® Portal access, if I am no longer employed at Baystate?
A: Any benefits-eligible employee, who has a job status change and is no longer benefits-eligible, will no longer have access to the portal. However, you can merge the health record stored in your portal with a WebMD.com consumer account, but you will have to do so while your WebMD® portal access is still active. Call Baystate Healthy at 413-794-4939 for further instruction.
Q: Can I access my WebMD® Portal from home?
A: Yes. Your entire portal, including the Health Questionnaire and Rewards page can be accessed from work or home at www.webmdhealth.com/bhy.
MyHealth Rewards Details and Eligibility
Q: What is MyHealth Rewards?
A: MyHealth Rewards is a voluntary incentive program designed to motivate and reward employees for taking an active role in managing their personal health and well-being and to help offset their health care costs. Read the wellness program notice of consent prior to participating in MyHealth Rewards.
Q: Who is eligible to earn the financial incentive?
A: All benefits-eligible employees can participate in this voluntary program.

Q: What is the Health Questionnaire?
A: The Health Questionnaire is a series of questions about your health needs, strengths and challenges. Your responses are combined to build your personalized wellness profile. It includes simple questions about lifestyle behaviors (such as diet, exercise, smoking, stress) and physical measurements (such as height, weight, cholesterol and blood pressure.) Once you complete your health screening, your physical measurements are sent securely from Interactive Health, Inc. to WebMD® and loaded into your Health Questionnaire for you. Taking the Health Questionnaire each year allows you to see your progress over time. It is also the “gateway” to the MyHealth Rewards program and therefore needs to be completed annually.
Q: How do I access and participate in the Wellness Activities? A: It is a little bit different for each activity. Go to the MyHealth Rewards page for program details.
Q: How do I participate in the ongoing raffle drawings?
A: Once you register for the WebMD® Portal and begin completing health activities, you will automatically be eligible for the ongoing raffles. The details of the drawings are provided within the portal. All benefits-eligible employees are eligible for the raffle drawings.
Q. Who is Interactive Health, Inc.?
A. Interactive Health, Inc. is a company that Baystate works with to provide onsite health screenings to employees. Onsite health screenings are offered at various locations throughout the system from January through June and are available of a first come first served basis.
Q. How do I schedule, modify or cancel an onsite health screening appointment?
A. To schedule, modify or cancel a health screening, go to www.hsscreeningreg.com. Appointments are available on a first come first served basis. Login: baystate (all lowercase) Password: healthy (all lowercase).
Q: After I complete a health screening, how soon will I see the reward I earned?
A: Allow about 2 weeks for your screening data to load in your WebMD portal. If you are concerned that your health screening data has not been sent to your WebMD account, call Interactive Health, Inc. at 1-800-711-8656. Keep in mind that along with all four health screenings tests, the Health Questionnaire must also be completed to earn any amount of reward.
Q: What happens to my health screening data?
A: Interactive Health, Inc. holds the health screening data and protects your confidentiality as described on the privacy agreement that you sign at the time of your health screening. Baystate does not see your individual information. Only an aggregate summary report is provided to help Baystate understand overall health needs of the employee population.
Q: What if I did not meet the Target Values set for biometric health screenings?
A: You earn rewards just by completing a health screening and the health questionnaire. If you did not meet the individually set Target Values, you can retest one additional time during the program year, through your health care provider, using the Physician Screening Results form.
Q: What if I think I am unable to meet a standard for a reward?
A: If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact Baystate Healthy at 413-794-4939 and we will work with you to find a wellness program with the same reward that is right for you in light of your health status.
Q: Does self-reported health screening data qualify for MyHealth Rewards?
A: No. To qualify for MyHealth Rewards, health screening data must be professionally-sourced and either provided through your health screening with Interactive Health, Inc. or your through your health care provider, via the Physician Screening Results form.
Earned Rewards and Payout
Q: What is the reward?
A: All active benefits-eligible employees who complete the minimum requirements and have all rewardable activities processed by October 31 can earn up to $500 by participating and meeting certain health screening target values.
Q: Will I receive confirmation telling me that I have completed MyHealth Rewards?
A: Yes. If you elected to receive email* from WebMD®, once you have completed the Health Questionnaire and a health screening, you will receive a confirmation email stating that you have completed the minimum requirements to earn Rewards. However, it is still your responsibility to verify your earnings captured on your rewards portal, prior to October 31. *WebMD uses the email address you used when setting up your account. This could be a work or personal email.
Q. How do I verify that I have earned rewards?
A. Log in to www.webmdhealth.com/bhy and review your Rewards page before October 31, 2017. We recommend that you check your account throughout the year, as you earn rewards. Only rewards captured in your Rewards account by October 31, 2017 will be paid to you. If you have a question about your account, complete and submit this form before the end of the program year.
Q. When will I receive my reward?
A. The reward will be distributed in the second paycheck in January 2019. You must have completed and have processed the required actions within the program year and be an active employee at the time the reward is distributed. If you did not receive the reward for the previous program year, in your second paycheck in January as expected, you must notify Baystate Healthy by January 31st. We are unable to process requests for corrections after this date. Per IRS guidelines, this wellness credit is considered taxable income and therefore all applicable taxes will be deducted from the total amount.
Q. What should I do if I do not get the reward I was expecting to receive?
A. If you verified your rewards account by the October 31 deadline, but your payout amount in paycheck is different than what is reflected in your account, you should complete and submit this form so that we can address any discrepancies.
If your earnings in your MyHealth Rewards account match the amount you received in your paycheck, but you did not report an issue with your account prior to the October 31 deadline, no further action can be taken. Only the rewards reflected in your account as of October 31 will be paid to you unless reported to us prior to October 31.
Q. If my job status changes to per diem, will I receive the reward I earned when I was eligible?
A. Yes, if you are an active employee at the time of payout and earned rewards while you were in a benefits-eligible status, you will receive the amount that you earned in your paycheck.
