Q: Which activities must I complete, in order to meet the minimum requirements to earn rewards?
A: If you choose to participate in MyHealth Rewards, you are required to complete the minimum requirements: the annual WebMD Health Questionnaire and an annual Health Screening. That health screening must include a measure of all 4 biometrics: glucose, BMI/Waist, HDL or LDL and glucose. The four additional Wellness Activities are optional.
Q: What is the Health Questionnaire?
A: The Health Questionnaire is a series of questions about your health needs, strengths and challenges. Your responses are combined to build your personalized wellness profile. It includes simple questions about lifestyle behaviors (such as diet, exercise, smoking, stress) and physical measurements (such as height, weight, cholesterol and blood pressure.) Once you complete your health screening, your physical measurements are sent securely from Interactive Health, Inc. to WebMD and loaded into your Health Questionnaire for you. Taking the Health Questionnaire each year allows you to see your progress over time. It is also the “gateway” to the MyHealth Rewards program and therefore needs to be completed annually.
Q. Who is Interactive Health, Inc.?
A. Interactive Health, Inc. is a company that Baystate works with to provide onsite health screenings to employees. Onsite health screenings are offered at various locations throughout the system from January through June and are available of a first come first served basis.
Q. How do I schedule, modify or cancel an onsite health screening appointment?
A. To schedule, modify or cancel a health screening, go to screeningreg.com, enter username: baystate, password: healthy and then follow the prompts on screen to sign-in. Appointments are available on a first come first served basis.
Q: Am I required to have my health screening done through Interactive Health, Inc.?
A: No. You may have your health screening done through your health care provider using the Physician Screening Results form. You are required to have all four screenings: blood pressure, body mass index, HDL or LDL cholesterol and glucose, unless you qualify for a reasonable alternative standard or an exception from you physician.
Q: After I complete a health screening, how soon will I see the reward I earned?
A: Allow up to 2 weeks for your health screening data to load in your WebMD portal. You will receive an email from WebMD indicating that professionally sourced data has been transmitted into your account. If you suspect that your health screening data was not successfully transmitted to your WebMD account, call Interactive Health, Inc. at 1-800-711-8656 to check on the status. Keep in mind that along with all four health screenings tests, the WebMD Health Questionnaire must also be completed in order to earn any amount of reward.
Q: What happens to my health screening data after I leave my onsite appointment?
A: Interactive Health, Inc. stores your health screening data and protects your confidentiality as described on the privacy agreement that you sign at the time of your health screening. Interactive Health will securely send your health screening data directly to your WebMD account and populate your rewards account, the Health Questionnaire and your WebMD health portal. Baystate does not see your individual information. Only an aggregate summary report is provided to help Baystate understand overall health needs of the employee population.
Q: What if I did not meet the Target Values set for biometric health screenings?
A: You earn rewards just by completing a health screening and the WebMD health questionnaire. You will also earn additional dollars for each Target Value that you meet. If you do not meet some or all of the individually set Target Values, you can retest one additional time during the program year, through your health care provider, using the Physician Screening Results form.
Q: What if I am unable to meet a standard for a reward?
A: If you think you might be unable to meet a standard for a reward under this wellness program, you may qualify for an opportunity to earn the same reward by different means. Contact Baystate Healthy at 413-794-4939 and we will work with you to find a wellness program with the same reward that is right for you in light of your health status.
Q: Can I self-report health screening data in order to qualify for MyHealth Rewards?
A: No. To qualify for MyHealth Rewards, health screening data must be professionally-sourced and transmitted to your WebMD account by Interactive Health’s secure transmission. Interactive Health will send data obtained through an onsite health screening or data recorded on a completed Physician Screening Results form.
Q: How do I access and participate in Wellness Activities?
A: It is a little bit different for each activity. Scroll up to the Wellness Activities section of this webpage for information about each rewardable option.
Q: How do I participate in the ongoing raffle drawings?
A: Once you register for the WebMD Portal and begin completing the MyHealth Rewards activities, you will be automatically placed in the raffle drawings. The details of the drawings are provided within the portal. All benefits-eligible employees are eligible for the raffle drawings.
Q: What is the reward?
A: All active benefits-eligible employees who complete the minimum requirements and have all rewardable activities verified in their WebMD portal by October 31 can earn up to $500 by participating in program activities and by meeting certain health screening target values. Reward earnings are added to employee’s paychecks and you must be an active employee at the time of payout in order to receive the payout.
Q: Will I receive confirmation telling me that I have completed MyHealth Rewards?
A: Yes. If you elected to receive email* from WebMD®, you will receive email confirmation once you have met the minimum requirements to earn rewards (completion of the WebMD Health Questionnaire and a health screening processed by Interactive Health, Inc.). The email will be from “WebMD-Baystate Healthy” and it will state that you have completed the minimum requirements to earn rewards and will instruct you to log into your WebMD portal to verify your earnings. This email is meant to confirm that you met the minimum requirements, only. It is your responsibility to verify that all of your completed activities have been successfully loaded into your rewards portal, prior to October 31. *WebMD uses the email address you providing during the registration process, when setting up your account. This may be a work or personal email address. You can view and/or update your preferred email in your WebMD Portal under Settings.
Q. How do I verify that I have earned rewards?
A. Log in to www.webmdhealth.com/bhy and go to the Rewards page (located on the homepage in the top menu bar). Click on Rewards History to view a time stamped confirmation of each earned reward. We recommend that you check your account throughout the year, as you earn rewards. Only rewards captured in your Rewards account by October 31, 2018 will be paid to you. If you have a question about your account, complete and submit this form before the end of the program year, so that we can help you address any questions or concerns about your earnings.
Q. When will I receive my reward?
A. The reward will be distributed to eligible earners in their second paycheck in January 2019. You must have completed and have processed the required actions within the program year and be an active employee at the time the reward is distributed. If you expect to receive this reward, but do not see it in your paycheck when scheduled, you must notify Baystate Healthy by January 31st. We are unable to process requests for corrections after this date.
Q. What should I do if I do not get the reward amount I was expecting to receive?
A. If you verify your earnings in your rewards account by the October 31 program deadline, but the amount in your paycheck in January is different than what is reflected in your account, you should complete and submit this form before January 31, so that we can address any discrepancies. If your earnings in your rewards account match what you receive in your paycheck and you did not report an issue prior to October 31, no further action can be taken. Also keep in mind that, per IRS guidelines, this wellness credit is considered taxable income and therefore all applicable taxes will be deducted from the total amount.
Q. If my job status changes to per diem, will I receive the payout, if I earned the rewards while I was benefits-eligible?
A. Yes, if you are an active employee at the time of payout and earned rewards while you were in a benefits-eligible status, you will receive the amount that you earned in your paycheck, as expected.
Q: If I earned MyHealth Rewards, but terminate employment prior to the payout date, will a separate check with my earnings be mailed to me?
A: If you are terminated due to a reduction in force (layoff), but already earned the reward during the program year, you will receive your payout. If you leave voluntarily or are terminated for any other reason, you are not eligible for payout.
