Baystate Hospice offers medical expertise through our extensive network of caregivers to support patients facing a serious or life limiting illness. This care team works together, with both the patient and family, to bring understanding, comfort, dignity and a sense of peace, as each patient journeys towards the final stage of life.
Baystate Pediatric Palliative Care program provides specialized care and comfort for children with life-limiting illnesses and their families. The care team’s goal is to improve the quality of life for the entire family.
Referring to Baystate
Referrals for our services from physicians’ offices can also be made by downloading our referral form and faxing everything directly to our office at 888-207-3106. Please be sure to include the patients’ demographics sheets, medication list, insurance information, and physician/advance practice provider office visit note supporting the need for this referral. You will receive confirmation from our intake team once we receive your referral.
Baystate Medical Practices can refer electronically to Baystate Home Health and Baystate Hospice directly from CIS.
